Information requested during your booking/order process
Your name, email address, phone number, and address.
I need these details to communicate run an effective service and issue your invoice.
Your invoice will show:

your preferred first name & last name, email address and phone number

a billing/delivery address (even if we're only meeting online)

a purchase order number if you added one when booking/ordering

the session/workshop title, date/s, and the cost OR the resources you've ordered with date and cost.
Your contact phone number
I only use this if we can't connect online.
Your workplace, occupation and any special requests or purpose for the workshop/session
This is confidential and usually optional. I only use it to prepare engaging and helpful content. In workshops, you can introduce yourself to others in whatever way feels right for you.
Your learning and accessibility needs
This is optional and confidential. I only use it to support your learning. If I can't meet your needs, I'll email you.
Information requested or obtained during at other times
Printed resource orders
I only store and use the delivery address you provide to send any printed resources you've ordered. Your delivery address, name, email address, phone number, and what you ordered will show on your invoice.
Information shared during workshops and all other sessions
We'll likely share a lot of information about each other's work and experience. This is all confidential so you can be open with me.
The only exception is if I believe someone is at risk of serious harm, in which case I will make very effort to talk it through with you before making an appropriate notification.
If I take notes during our sessions, these are kept in my locked home office or secure online storage. I archive/destroy my notes regularly.
After session feedback
I request your feedback after all workshops and after some sessions. It's always optional. I use your feedback to improve my services.
You may be invited to give permission for me to use your feedback to market my services. I'll need a display name & occupation and an optional selfie/image, but I won't store this information on your account. Giving permission is completely optional and won't impact the service quality I provide to you.
Information shared with others during your workshop
At the start of our workshop we discuss support each other and specifically what we mean by confidentiality. I encourage everyone to be open, but you do not have to share anything you don't want to.
You can set your own name and pronouns on Zoom (unless you're using an employer account that prevents you from doing so).

Workshops are never recorded and closed captions/whiteboards/chat exchanges are not saved unless the whole group expressly agrees for a good reason (e.g., accessibility).
HSRs only: information requested when formalising your US29315 achievement
If you attend my initial HSR training workshop and pass your assessment for Unit Standard 29315 I will request that you provide further detail to formalise your achievement with NZQA. Some of this information is compulsory and some is optional.

All information you provide through this form is shared with the three agencies that ensure your achievement is correctly assigned to your own NZQA 'account': WorksafeReps, iQualify/The Open Polytechnic, and NZQA itself. Your information may take a couple of months to be entered into the NZQA system. Contact me if you have any questions.
Information relating to 'in-house' training workshops
If you're coming to a workshop designed or contracted for your own workplace or industry, I will tell you which information will be passed on to your employer. ​
This will most likely be restricted to

whether you attended the workshop

whether you stayed for the duration of the workshop

whether you have passed any assessment (e.g., US29315 for HSRs)​​​
Unless you explicitly ask me to, I will never share with your employer/PCBU

your specific learning or accessibility needs

issues you've raised during our workshop or other communications

any additional data (e.g., marketing content or HSR unit standard US29315 data).
